OMNI is the grant evaluator for Virginia's PFS grant which sought to leverage resources and funding at the state and local levels for prevention disparities.

The data in this report provide both a snapshot of the status of efforts in PFS communities at the close of the grant as well as changes in outcomes over the life of the grant.
