Lincoln County Public Health’s Community Health Assessment, Capacity Assessment, and Community Health Improvement Plan
OMNI is proud to share recently released Community Health Assessment and Community Health Improvement Plan reports from Lincoln County, CO. These reports highlight the health assessment process, share valuable community data, and present key insights from community discussions, all reflecting OMNI's dedication to supporting and empowering communities to improve their community health.
OMNI recently partnered with Lincoln County Public Health to conduct their 2024-2029 Community Health Assessment (CHA) and corresponding strategic Community Health Improvement Plan (CHIP) in accordance with the Colorado Health Assessment and Planning System (CHAPS) requirements.
Through this work, we engaged in interpretation of qualitative and quantitative data collected by LCPH, as well as secondary regional and statewide data. We also facilitated priority setting for the county’s CHIP, focusing on potential partnerships, community capacity, buy-in, and readiness to change. Finally, OMNI supported LCPH to identify concrete activities that the community can engage in during the next five years to achieve their goal of improving access to care for all community members. The recently released report provides insights into the health assessment process, data obtained from surveys conducted among youth and adults, and insights gathered from interviews with community members.
